When evaluating diagnosis documentation for Inpatient Falls, I have the following questions as to how these should qualify in the following scenarios:

1. If a diagnosis is documented as a "working" diagnosis, meaning that the diagnosis is not finalized just a diagnosis that has potential relevance to a patient not yet confirmed, should that be considered as a valid diagnosis for logic analysis?
2. If a diagnosis is documented in an application or system that does not allow POA to also be documented, should that application be a valid system to be used in diagnosis evaluation of the measure logic?
