Thank you for your inquiry regarding CMS2v15. The document included in your issue description refers to the CQM, questions for which should be directed to the QPP Helpdesk at QPP@cms.hhs.gov. For the purpose of reporting data for the eCQM, per the measure specification (https://ecqi.healthit.gov/sites/default/files/ecqm/measures/CMS2-v15.0.000-QDM.html), an age-appropriate, standardized, and validated depression screening tool must be used for numerator compliance. The measure’s Definition section provides examples of acceptable tools (e.g., PHQ-2, PHQ-A); however, it is not prescriptive, and providers are encouraged to use their clinical judgment to select the appropriate tool for their patient population.

In reviewing the documentation, we see the list of Examples of standardized depression screening tools, which is not exhaustive, that the PHQ -9 and PRIME MD PHQ-2 are acceptable data collection tools. Our question is whether PHQ-4 is also an acceptable data collection tool for purposes of meeting the data collection requirements of this measure.

We would need to change our clinical operations to include a two different screeners if PHQ-4 isn't an acceptable data collection tool, which includes retraining our clinicians and asking our patients to fill out two screens vs one (one for depression, one for anxiety)
