Confimation of appropriate LOINC codes for CMS measures 871/816

      Thank you for your question regarding CMS816: Hospital Harm - Severe Hypoglycemia and CMS871: Hospital Harm - Severe Hyperglycemia. When evaluating glucose test results, both measures use the ["Laboratory Test, Performed": "Glucose Lab Test Mass Per Volume"] Quality Data Model (QDM) data element, which uses codes in the “Glucose Lab Test Mass Per Volume” value set (https://ecqi.healthit.gov/mcw/2026/ecqm-dataelement/laboratorytestperformedglucoselabtestmasspervolume.html). More information on the 16 LOINC codes included in the "Glucose Lab Test Mass Per Volume" value set (OID: 2.16.840.1.113762.1.4.1248.34) can be found on the Value Set Authority Center (VSAC) (https://vsac.nlm.nih.gov) or in the VSAC downloadable files for the 2026 reporting period (https://vsac.nlm.nih.gov/download/ecqm). Contact with the Office of the National Coordinator for Health Information Technology (ONC) is not required to validate the LOINC codes included in eCQMs. Note that LOINC code 14743-9 is NOT included in the value set for the two measures.

      Good afternoon, looking to get some guidance and insight into the reccomeneded LOINC codes associated with Measures CMS 871 and CMS 816. Per the training we have received from our EHR company, we are to source and validate these codes for approved use through contact with the ONC. Please let me know if I do not have the correct agency and apologies pre-emptively. The specific LOINC codes in question for use with the above measures with regards to glucose are: 41653-7, 2345-7 and  14743-9. If these codes are not the approved codes for the above measures, can you please cite or point me in the direction to view those codes and thier associations. Thank you.
