Guidance for submitting medication administration dosage

      We need guidance on how to represent medication administration dosage in the QRDA submission file. For example, if a patient is prescribed 5 mg of warfarin during hospitalization, but the pharmacy only has 1 mg capsules available, resulting in the patient receiving five 1 mg capsules, how should this be recorded in the QRDA file?
      Should we report a total administered dose of 5 mg, or should we report one administration with a 1 mg product and either a dose of '5 mg' or '5 tabs'?

      We would also like guidance on IV doses. For example, the product is 1000 UNT/mL heparin with 1 mL. Only .9 mL is administered. Do we submit .9 mL, 900 UNT, or just .9 (as in 9/10ths of the product).
