We are unable to provide specific guidance related to internal EHR configuration, historical data loads or local medication mapping and documentation practices. We recommend you continue to consult with your EHR vendor and clinical partners. If your organization performs internal mapping or remediation activities to address these issues, we recommend maintaining clear documentation of the decisions, assumptions, and validation steps taken, in the event of a CMS audit.

We ran a historic load to update and capture patients administered MOUD medications. This update removed patients who would otherwise fall into the measure. Our EHR vendor recommended another historical load, which removed more patients. Can you provide guidance on this? We want to ensure accurate reporting for the 2025 submission and feel that our IPP/Denominator population is not capturing all patients prescribed or continued on qualifying med at discharge.
