For Depression Screening and Follow- up (CMS 134), as we move to primary eCQM reporting, our provider governance group approved and defined the following drop down menu to document the follow-up plan for positive depression screens.  We are requesting guidance on appropriate assignment of SNOMED codes to map these provider follow up planning interventions. The defined intent for each of the menu selections outlined below were communicated to providers with direction to expand upon plan elements within narrative notes: 

1. Clinic Follow up  (SNOMED CT 410231007,Mental health screening management-procedure): Use this option for clinic follow up at a future date to reassess symptoms
2. Continue current treatment (SNOMED CT 410234004,Management of mental health treatment -procedure ):Use this option if no current changes to plan of care at this time
3. Referral to Behavioral Health (SNOMED CT 381294000,Patient referral for psychotherapy-procedure): Use this option when referring to behavioral health service,e.g. psychotherapy, mental health team
4. Referral to Counseling (SNOMED CT 306226009, Referral to mental health team-procedure): Use this option when referring to counseling services
5. Referral to Spiritual Care ( SNOMED CT 711281004,Referral to support group-procedure): Use this option when referring to spiritual-based counseling or support services
6. Prescription Medication (SNOMED CT 410234004, Management of mental health treatment -procedure): Use this option when prescribing medications to improve mental health
7. Patient Education (SNOMED CT 410224008, Mental health care education (procedure):  Use this option when providing education to improve mental health
8. Other- (SNOMED CT 410231007, Mental health screening management (procedure) : Use this option when implementing actions based on clinical judgement to improve mental health (specify details in free text).
