      Thank you for your inquiry regarding CMS871v4, Hospital Harm - Severe Hyperglycemia. For this measure, the inpatient hospitalization period includes time in the emergency department (ED) and/or observation status when the transition between discharge from these encounters and the start of the inpatient encounter is one hour or less. In the screenshot you shared, we are not able to identify which event update is recorded as the ED encounter end time in your facility's electronic health record (EHR) system. However, if there is more than one hour between the ED encounter end and the start of the inpatient encounter, then the inpatient hospitalization period assessed by this measure would begin at the start of the inpatient encounter. But if there is one hour or less between the ED encounter end and the start of the inpatient encounter, then the inpatient hospitalization period assessed by this measure would begin at the start of the ED encounter.

      Can you please clarify the encounter start date/time for one of our cases. See attached event timeline for reference. In the example provided, should the encounter start date/time for the measure begin at the ED arrival time on 8/11 1506 or admission at 8/11 1611.
