Thank you for your inquiry regarding CMS816: Hospital Harm – Severe Hypoglycemia. A patient does not qualify for the numerator if there is a repeat test for glucose with a result greater than 80 mg/dL within five minutes or less from the start of a glucose test with a result less than 40 mg/dL. Since the repeat blood test in your example is over 80 mg/dL and within 5 minutes of the original result, this patient should not qualify for the numerator.

A severe hyperglycemic event is defined by:

      - A glucose test with a result less than 40 mg/dL

      AND

       - A hypoglycemic medication was administered within 24 hours before the start of the severe hypoglycemic event (i.e., the glucose test with a result less than 40 mg/dL)

      AND

       - There was no subsequent repeat test for glucose with a result greater than 80 mg/dL within five minutes or less from the start of the initial glucose test with a result less than 40 mg/dL
      Show

We need clarification on the following case, patient had a POC blood glucose of 22 mg/dl on 9/2/25 at 4:49:52 pm followed by a repeat blood glucose of over 80 mg/dl on 9/2/25 at 4:54:01 pm
The patient was in the numerator. However, because the repeat occurred just within the 5-minute mark of the initial low reading, would you consider this patient to be counted in the denominator instead?
