    • eCQM data is pulling diagnosis codes that are entered by non-qualified care team members from the ambulatory setting

      Problem List for healthcare systems can be populated via the inpatient and ambulatory charting workflows. This means that codes may be entered by clinical care team members other than doctors. These team members are not qualified to diagnose patients, yet their additions to the Problem List are included in eCQM measures.
