      Thank you for your question regarding Venous Thromboembolism Prophylaxis (CMS108) and Intensive Care Unit Venous Thromboembolism Prophylaxis (CMS190). Regarding the consideration of ICD-10 code I51.3 – Intracardiac thrombosis, not elsewhere classified for inclusion in the value set “Atrial Fibrillation or Flutter, VTE” (2.16.840.1.113883.3.117.1.7.1.202). The purpose of this value set is to represent concepts of diagnoses to identify patients with a history of atrial fibrillation/flutter or a current finding of atrial fibrillation/flutter. Because intracardiac thrombosis can occur independently of atrial fibrillation or flutter, the diagnosis code I51.3 does not meet the intent of the value set designed specifically to identify AF or atrial flutter.

      Consider adding Diagnosis Code I51.3 under 'Value Set Names: Atrial Fibrillation or Flutter, VTE' in the VTE value set for both VTE1 and VTE2.  Diagnosis I51.3 should be included to allow administration of Oral Factor Xa Inhibitor as an allowed medication for VTE prophylaxis. 

      "Encounter with Medication Oral Factor Xa Inhibitor Administered on Day of or Day After Admission or Procedure" should union w/ "Encounter with Prior or Present Diagnosis of Atrial Fibrillation or Prior Diagnosis of VTE" with the addition of this Diagnosis code to meet Numerator

      "Encounter with Medication Oral Factor Xa Inhibitor Administered on Day of or Day After First ICU Stay or Procedure" should union w/ "Encounter with Prior or Present Diagnosis of Atrial Fibrillation or Prior Diagnosis of VTE" with the addition of this Diagnosis code to meet Numerator
