Thank you for your question about CMS1028v4/PC-07- Severe Obstetric Complications. N17.9, Acute kidney failure, unspecified, is included in the severe maternal morbidity (SMM) value set which also considers the POA indicator for inclusion in the numerator. A POA indicator of N (No) or UTD (Unable to Determine) is required for SMM diagnoses to qualify for the numerator.

We are seeing an increase in cases for CY 2025 fall into the numerator for N179. Any documentation that contains AKI is being coded at N179 ( Acute Kidney  Failure)  and then being pulled into the numerator for PC07 under Acute Renal Failure. Can you confirm if this is a maternal morbidity diagnosis  (N179 Acute Kidney Failure)  and it is correct for pulling in under Acute Renal Failure in the value set for this measure?
