We are reaching out to Imaging Facilities for reports on patients that have claims of a mammogram and fear that this will not be uploaded to the EMR in time
I am reaching out with a question regarding acceptable documentation timing for the Breast Cancer Screening measure under MIPS/QPP.
If a patient completed a mammogram within the applicable lookback measurement period, but the imaging report was not scanned or uploaded into the medical record until after 1/1/2026, can this delayed upload still be used as valid evidence for meeting the measure—assuming the date of service for the mammogram clearly falls within the required lookback period? We were informed in a spotlight call during reporting season last year that, yes, it could be since the evidence is based on the date of service but want to make sure this is still the case.
To clarify, the service itself occurred during the measurement timeframe, but the documentation became available in the EMR after the start of the subsequent reporting year.
Can you please confirm whether:
- The date the mammogram was performed is the determining factor for measure compliance, regardless of when the documentation is entered into the EMR, or
- The documentation must be present in the record before 1/1/2026 to be eligible for reporting?
