Clarification on Abnormal Presentations

      Thank you for your question about CMS334/PC-02 Cesarean Birth. A compound presentation occurs when a fetal extremity presents alongside the leading cephalic or breech part. Some compound presentation codes are included in the value set, such as O64.8x, which is applicable for footling and incomplete breech. Codes such as those for occiput posterior are not in the value set. These are vertex positions that can be delivered vaginally and/or may be corrected with labor maneuvers.

      Cord presentations are not excluded, except O69.4x vasa previa. Our Perinatal Technical Advisory Panel (TAP) felt it is too broad and is often a complication caused by labor management.

      Concerning CMS 334 Ceasarean Birth, I would like clarification on if cord and compound presentations are part of the abnormal presentations excluded for this measure. In reviewing the VSAC codes and specs for abnormal presentation 2.16.840.1.113762.1.4.1045.105 , cord and compound are not listed, however per clinical guidelines we understand compound and maybe cord is an abnormal presentation. Question is, are cord and compound presentations abnormal presentations that should be excluded for the PC-02 measure?
