      Thank you for your inquiry regarding CMS819v3 and CMS819v4, Hospital Harm - Opioid-Related Adverse Events. Since Fentanyl was administered in the operating room, it does not meet the denominator and initial population where qualifying encounters include those with at least one opioid medication administered outside of the operating room. Therefore this patient should be excluded from the denominator.

      I have a patient that arrived to the ED on 3/1/2025 00:07, was admitted as observation status on 3/1 04:36, went into the OR 3/1 12:24, received fentanyl 100 mcg in total while inside the OR 12:52, received the reversal agent Narcan while inside PACU 3/1 14:15, then later on the patient was transferred to inpatient status on 3/2 08:44. Due to the fact that the patient not inpatient and the opioid was administered during the OR suite encounter, should this patient be excluded from the denominator for this measure?
