The measure’s Guidance section states that “The documented follow-up plan must be based on the documented BMI, outside of normal parameters.” The definition section also provides examples of follow-up plan, which includes activities such as exercise, nutrition, counseling or education, medication, and surgical interventions. Please note the follow-up plan must meet the data element criteria specified within the value sets for Follow Up for Above Normal BMI (2.16.840.1.113883.3.600.1.1525) or Follow Up for Below Normal BMI (2.16.840.1.113883.3.600.1.1528). If the documentation provided in Epic can be mapped to the one of the codes in the value set, then the numerator requirements would be satisfied.

      NCQA received this inquiry about the CMS69v13 (Body Mass Index (BMI) Screening and Follow-Up Plan), which is used in the CA-QIP program. Does the following satisfy the numerator requirement for Follow- up plan even though it doesn't specifically mention the abnormal BMI? This flow sheet is completed in Epic when a BMI is evaluated out of range, this flow sheet prompts for the providers and can be found in the visit notes.

      Patient Counseling:
       Nutrition: Stressed importance of moderation in sodium/caffeine intake, saturated fat and cholesterol, caloric balance, sufficient intake of fresh fruits, vegetables, fiber, calcium, vitamin D, iron.
       Discussed the issue of calcium supplement, and the daily use of sunscreen to minimize exposure to Ultraviolet (UV) radiation and to reduce the risk of skin cancer.
       -Exercise: Stressed the importance of 150 minutes of moderate intensity or 75 minutes of vigorous intensity exercise per week.
       Substance Abuse: Discussed cessation/primary prevention of tobacco, alcohol, or other drug use; driving or other dangerous activities under the influence; availability of treatment for abuse.
       Sexuality: Discussed sexually transmitted diseases, partner selection, use of condoms, avoidance of unintended pregnancy and contraceptive alternatives.
       Injury prevention: Discussed safety belts, safety helmets, smoke detector, smoking near bedding or upholstery.
       Dental health: Discussed importance of regular tooth brushing, flossing, and dental visits.
       Immunizations reviewed.
       Discussed and educated on age appropriate health maintenance and cancer screenings.
       After hours service discussed with patient
