Reasons for not prescribing antithrombotic therapy or a reason for no antithrombotic order or administered must be documented by a physician/APN/PA or pharmacist, with one exception: Patient/family refusal of any form of antithrombotic therapy (e.g., “ASA refused,” “Patient refusing antithrombotic therapy”) may be documented by a nurse.
      For CMS71 (STK3), the denominator exception is looking for if a reason for not prescribing anticoagulation therapy was documented for patient discharge, as the "Medication, Not discharge" data element is specified in the definition. But the documentation time could be anytime during this encounter.

      For CMS72 (STK-5), the denominator exception is looking for if a reason for no antithrombotic order or administered was documented on the day of or day after hospital arrival, as " Medication, Not Ordered" and " Medication, Not Administered" data elements are specified in the logic. And the documentation time could be anytime during the day of or day after hospital arrival.

      I am looking for clarity as to whether or not the exception related to anticoagulation not prescribed at discharge and/or not given during the encounter must be documented by a Physician/Provider or if it can be documented by a nurse?
