      The patient had Eliquis at discharge, but there is no dosage noted in the DC Med Req, so this medication is currently not being mapped in EPIC. Does the antithrombotic require a dosage to count for this measure?

      If the medication is continued from admission, can the dosage of the admit med be used for the discharge antithrombotic they will continue on?
