      Thank you for your question regarding CMS506v7, Safe Use of Opioids - Concurrent Prescribing. Patients discharged to settings where they are not under direct close medical management and are on multiple opioids are at risk for complications, although this may be appropriate medical management for some patients. The goal is not to score 0% of patients being discharged on multiple opioids or and opioid and benzodiazepine for this measure, rather the intent is to capture patients who are prescribed either of these combinations because they put patients at risk for complications. We do appreciate your feedback, however currently, there are no plans to exclude patients with home medications from this measure.

    • Patients receiving ongoing outpatient palliative care are not excluded from measure.

      Patient admitted and treated for acute ischemic stroke.  Medication list shows patient on long-term opioid and benzos managed by outpatient palliative care for patients COPD and Heart Failure.  At discharge, provider reviews medication list and does not make any recommended changes to the outpatient palliative care opioid/benzo prescriptions.  Provider believes this patient should be excluded from the measure.
