       - If any blood transfusion occurs and is coded (SNOMED-CT/ ICD-10) during the delivery encounter, it will count towards this measure's numerator 1. However, if it is the only severe obstetric complication, the encounter will not qualify for numerator 2.
       - N17.9, Acute kidney failure, unspecified, is included in the severe maternal morbidity (SMM) value set which also considers the POA indicator for inclusion in the numerator. A POA indicator of N (No) or UTD (Unable to Determine) is required for SMM diagnoses to qualify for the numerator.

      2 Questions:

      • If a patient presents with anemia and receives a blood transfusion prior to delivery, should this intervention be counted in the numerator as a complication? Currently, EPIC appears to be flagging these cases as complications, even when the transfusion is administered in response to a pre-existing condition rather than as a result of an adverse event related to labor or delivery.

       

      • In a recent case, the provider documented Acute Kidney Injury, which was subsequently coded by the coding team as N17.9 – Acute Kidney Failure. This coding led to the encounter being flagged as a complication for the measure.

      Per our coding team Acute Kidney Injury codes to N17.9 Acute Kidney Failure and Acute Kidney Failure is listed as complication for the measure.
