The specifications state that patients discharged to other acute inpatient facilities, as defined in the value set "Discharge To Acute Care Facility" (OID 2.16.840.1.113883.3.117.1.7.1.87), are excluded. This value set includes discharge to community hospitals, tertiary referral hospitals, and acute care hospitals. Patients transferred to a rehab, psychiatric hospital, etc. are not excluded.

      The value set notes only three options apply: community hospital, tertiary referral center or general acute care facility. So my question is do these three options align with value 4 for chart abstracted measures, which means that discharge to a psychiatric hospital would not be considered acute care facility?
