QM317 clarification of requirement for referral documentation

      Thank you for inquiring about the Preventive Care and Screening: Screening for High Blood Pressure and Follow-Up Documented eCQM. Note that the measure specification requires documentation of the order with direct linkage to the referral reason being an abnormal blood pressure. Verbal discussion alone, even if documented in a clinical note, does not meet the measure criteria. Refer to the version 14 specification logic excerpt below:

      I was hoping to get clarification re: QM317.  When it says "Referral to Alternate/Primary Care Provider" (as an option for recommended follow up for an elevated BP reading), does that mean that an actual referral order has to be generated electronically?  Or is it adequate to verbally refer the patient to the alternate/primary care provider and document doing so?
