The measure developer does not specify a pathway to identify documentation of “Not Malnourished” or “Well Nourished”. This grants implementers the flexibility to establish their own workflows in accordance with their specific operational resources. We recommend reviewing the Implementation Checklist, available at www.ecqi.healthit.gov/ecqm-implementation-checklist. Step 4 of the Implementation Checklist offers suggested steps. In addition, please coordinate with your EHR vendor to verify that your system is configured correctly to capture the necessary documentation for this measure.

      Most RD's chart by exception and do not document " Not malnourished or Well nourished" especially if the patient has other nutritional problems that impact status. If the Malnutrition risk is triggered as at risk and the RDN performed an assessment and does not find the patient as Moderate or Severe malnourished then these patients are falling out as non compliant since they did not document with the above terminology
