The eCQM measures are designed to use clinical data from structured fields in the EHR, rather than billing or claims data. Please note that the codes LA34398-0 and LA34399-8 are included in the valueset "Autonomous Eye Exam Result or Finding" (2.16.840.1.113883.3.464.1004.2616). In addition to meeting numerator criteria for an autonomous eye exam in the measurement period, a physical exam must be performed during the measurement period using LOINC Code (105914-6). Clinically equivalent services can be mapped to the measure's value set or codes. If mapping is done, please maintain documentation in case of a CMS audit.
