      Thank you for your question regarding CMS334v6, Cesarean Birth. While extensive or complicated myomectomy can be an indication for a cesarean section, these cases are rare in nulliparous patients. The code to identify prior myomectomy is too broad to only exclude extensive or complicated cases, which is why it is not included for the exclusions for the measure. As always, we recommend that the appropriate care be provided based upon the provider’s assessment of the patient’s needs, regardless of the impact on the performance calculation.

      I only see that abnormal presentation, genital herpes, placenta previa, vasa previa, and placental accreta spectrum are the only exclusions of this measure.  Wouldn't previous history of uterine scar in the contractile portion of the uterus with a myomectomy be excluded from this measure, especially since a history of uterine surgery puts the mother at high risk for uterine rupture?
