Thank you for inquiring about the Documentation of Current Medications in the Medical Record eCQM. While the reporting eligible clinician is the one who gets credit for documentation of current medications, the review can be completed by any type of provider (e.g., MA, nurse, pharmacist) during the qualifying encounter. Your EHR implementation correctly reflects the measure’s intent.

    • MIPS Eligible Clinicians are meeting the measure even when they do not attest the medications during the encounter.

For CMS 68, the criteria states "Eligible clinician attests to documenting, updating, or reviewing the patient's current medications using all immediate resources available on the date of the encounter." However, in Epic currently the numerator is built to capture anytime the medications are reviewed during the date of the encounter by any clinician (medical assistant, nurse). Does this mean that if the nurse or medical assistance reviews the medications on the date of the encounter, the MIPS eligible clinician will get credit for meeting the measure for that date of service?
