HH AKI issue with not knowing heart failure, diabetes, obesity, cancer, and hypertension diagnoses

This measure does not specify particular value sets in the logic because it requires reporting of all encounter diagnoses with Present on Admission (POA) indicators—not just those related to cancer, hypertension, diabetes, heart failure, and obesity. Please submit all encounter diagnoses with POA indicators for all reported patients. We will consider whether value sets inclusive of the diagnosis-related risk adjustment data elements with ICD-10 and POA codes are needed in a future Annual Update.

      When the measure asks for the cancer, hypertension, diabetes, heart failure, and obesity diagnoses, there is nothing that I can find in VSAC, that tells me that I am viewing what is implicated by these diagnoses as it relates to codes. Usually, there are codes for these in VSAC. I am unable to find those. Based on the measure specifications, is it referring to any, meaning "any" and "all" of ICD-10-CM codes that related to those conditions?

       

      Desired outcome entered in the Solution field: Please put the referred to diagnoses in VSAC for cancer, hypertension, diabetes, heart failure, and obesity.
