      Please help me to understand how to calculate observation 6 and get the score.  Below is the breakdown for our pt:

      MO1: has screening, not at risk; also has referral to dietitian (this =1)

      MO2: has assessment, result = moderately malnourished (this =1)

      MO3: has qualified malnutrition diagnosis (this =1)

      MO4: has nutrition care plan (this = 1)

      MO5: would be 4

      MO6: since the pt was screened as not at risk and also had a dietitian referral, would the "total malnutrition composite score eligible occurrences" be 2 based on the explanation below making the equation 4/2 = 2 X100 = 200%.  I know the max score should be 100%.  Please help clarify if the "total malnutrition composite score eligible occurrences" should be 2 or 1.
