The measure looks for diabetic patients aged 18-75 who had an outpatient visit during the measurement period and received a retinal or dilated eye exam, based on whether they had retinopathy diagnosed during the period or within the previous year. Documentation should include exam details, date, results, and confirmation by an ophthalmologist or optometrist. Exams conducted externally must be recorded in the EHR using a SNOMEDCT code from the "Retinal or Dilated Eye Exam" value set. However, external results not documented in the EHR will not satisfy the measure criteria, regardless of where the exams were performed.

      If a practice receives referrals for patients with diabetes and the dilated exam was performed by an outside TIN clinician, could the practice create a non-billable encounter with the dilation info, similar to a co-managed cataract patient — and still qualify for the numerator? See below from the spec sheetNumerator info: * Patients with an eye screening for diabetic retinal disease. This includes diabetics who had one of the following:

        • Diabetic with a diagnosis of retinopathy in any part of the measurement period and a retinal or dilated eye exam by an eye care professional in the measurement period.
        • Diabetic with no diagnosis of retinopathy in any part of the measurement period and a retinal or dilated eye exam by an eye care professional in the measurement period or the year prior to the measurement period.
