      Thank you for your question regarding CMS506v7. We appreciate your comments and questions. Patients discharged to settings where they are not under direct close medical management and are on multiple opioids are at risk for complications, although this may be appropriate medical management for some patients. CMS recognizes this and the goal is not to score 0% of patients being discharged on multiple opioids or and opioid and benzodiazepine for this measure, rather the intent is to capture patients who are prescribed either of these combinations because they put patients at risk for complications. We do appreciate your feedback, however currently, there are no plans to exclude surgical hospitals from this measure.

      The value set 2.16.840.1.113762.1.4.1046.241 for Opioid Medications - Medication: Schedule II, III and IV Opioid Medications has additional meds (schedule IV) included in for data collection in 2025. This change has caused a significant increase in Numerator qualifiers for our Surgical Hospitals. Some are seeing their performance rates jump up 40% or more than 2024). As this measure is significant (publicly reported, STAR ratings), is there any consideration for exclusion of surgical hospitals, patients, etc.?
