      Thank you for your question regarding VTE-1 CMS108. Yes, patient refuses intervention and documented as “SCDs off. Pt refuses interventions" or “Pt refuses intervention” would be considered sufficient to meet the numerator condition of "Encounter with No VTE Prophylaxis Due to Patient Refusal".

      Under a section of charting labeled VTE Prevention/ Management, documentation states "SCDs off.  Pt refuses intervention."  Can it be assumed that the intervention refused was the SCDs or does the documentation need to be more specific?  What about if under the same section, VTE Prevention/ Management, it just says "Pt. refuses intervention." Does that assume mechanical and pharmacologic interventions were both refused?
