Thank you for your question regarding CMS826v2 and CMS826v3, Hospital Harm - Pressure Injury. For this measure, the inpatient hospitalization period includes time in emergency department (ED) and/or observation status when the transition between discharge from these encounters and admission to the inpatient encounter is one hour or less. Using the example you provided, if the patient is discharged from the ED at 12:15PM and is admitted to the inpatient encounter at 1PM, then the inpatient hospitalization period assessed by the measure would begin at 10AM (the ED encounter start time), as the transition between discharge from the ED encounter and the start of the inpatient encounter is less than one hour. If the patient is discharged from the ED at 11:45AM and is admitted to the inpatient encounter at 1PM, then the inpatient hospitalization period assessed by the measure would begin at 1PM (the inpatient encounter start time), as the transition between discharge from the ED encounter and the start of the inpatient encounter is more than one hour.

    • What is the admit start time

Reaching out to clarify when the admit time starts for the measure from when the patient is in the ED or observation or when the patient becomes an inpatient? For example, if a patient is in the ED at 10 am and then is admitted inpatient at 1 pm. Does the admit time start from 10 am or 1 pm?

Measure includes inpatient admissions who were initially seen in the emergency department or in observation status and then were admitted to the hospital.
