      Hello, Thank you for question regarding CMS71 STK-3. If a diagnosis of Atrial Fibrillation is documented on a previous visit/ current visit, or is documented as a history of it is considered applicable to the current encounter.

      The logic checks whether the Atrial Fibrillation/Flutter (AF) diagnosis start time occurred on or before the Ischemic Stroke encounter. Once a patient has AF, they are always at risk. The nature of the arrhythmia is that it comes and goes, e.g., “paroxysmal”. It can also be persistent/permanent. Even with patients that have ablation procedures, it is not uncommon for AF to return.

    • Exclusion denominator for STK 3. Pt does not discharge with anticoagulant. because patient has no AFIB in Problem list where it was coded/pulled but another textual document of Physical therapist

      To meet the Denominator exclusion, what is the acceptable element in EHR, pulled from  textual document/form of a Physical therapist/DC Summary  back in 2018. not in patient problem list nor coded as POA diagnosis. It was problem list cancelled stricken through in 2019, 2022. It was not coded nor listed in patient current encounter. My question is what is the timeframe for historical pulling of diagnosis/problem list. Maybe pt do not have AFIB anymore, the reason it was not ordered anymore. Please advise.
