      Thank you for your question regarding CMS1218v2, Hospital Harm - Postoperative Respiratory Failure. This measure assesses the number of elective inpatient hospitalizations for patients aged 18 years and older without an obstetrical condition who have a procedure resulting in postoperative respiratory failure. As such, the logic used for this measure’s initial population uses the “Elective Inpatient Encounter” value set (2.16.840.1.113762.1.4.1248.85) to identify specifically elective inpatient hospitalizations that may qualify for the initial population. The “Elective Inpatient Encounter” value set contains the four SNOMED CT codes for elective hospital admissions, refer to the attached table.

      The intent of the “Encounter Inpatient” value set (2.16.840.1.113883.3.666.5.307), used in other hospital eCQMs, is to identify both elective and non-elective hospital admissions. Therefore, the “Encounter Inpatient” value set contains three SNOMED CT codes for elective and non-elective hospital admissions, refer to the attached table.

      As you can see, both the “Elective Inpatient Encounter” and “Encounter Inpatient” value sets contain one common code for elective hospital admissions: “Hospital admission, elective (procedure)”. The “Elective Inpatient Encounter” value set contains three additional codes for elective hospital admissions, which are descendant codes of the "Hospital admission, elective (procedure)" code and are not included in the “Encounter Inpatient” value set. This may result in certain encounters meeting the CMS1218v2 Initial Population criteria that may not meet the Initial Population criteria of other hospital eCQMs that use the “Encounter Inpatient” value set to determine potentially qualifying inpatient encounters.

      The value set stewards of both value sets may consider future updates to the value sets.

    • We need a clear understanding of the intent of the new Elective Inpatient Encounter value set and what is expected for existing measures that use the "Encounter Inpatient" value set. We are currently blocked with developing the HH-RF measure.

      The values in the Elective Inpatient Encounter value set appear to represent more specific versions of the Inpatient encounter type. Could you clarify why these codes are included in the Elective Inpatient Encounter value set but not in the existing Inpatient value set? Additionally, should encounters be expected to qualify under the Elective Inpatient Encounter value set but not the Inpatient value set? If not, would multiple encounter codes be required on the file? If multiple codes are expected, this may introduce inconsistencies with prior measure specifications (VTE,HH-Hypo/Hyper, etc.), and we would like to confirm whether this outcome is intentional.
