      Thank you for your follow-up inquiry regarding CMS996: Appropriate Treatment for ST-Segment Elevation Myocardial Infarction (STEMI) Patients in the Emergency Department (ED). Updated Version 6 STEMI measure specifications for the 2026 reporting period have been posted to the eCQI Resource Center: https://ecqi.healthit.gov/ecqm/hosp-outpt/2026/cms0996v6?qt-tabs_measure=measure-information. These updated measure specifications include denominator exceptions for ED encounters with a documented reason for not administering fibrinolytic therapy within 30 minutes of ED arrival or (ii) not performing a PCI within 90 minutes of ED arrival.

      Our facility has had multiple patients fail to meet the numerator for OP-40 due to presenting with STEMI, proceeding to cath lab, and requiring conversion to emergent/urgent CABG. These patients are treated appropriately for their clinical condition and should be excluded from the denominator.

These patients are treated appropriately for their clinical condition and should be excluded from the denominator.

      Patients presenting with STEMI but requiring emergent/urgent CABG rather than PCI/thrombolytics should be considered from the OP-40 denominator.
