We are using Practice Fusion to report for MIPS. Our concern is that some patients have only visited the practice once and are now inactive, while others are deceased. Is there any form of exemption available for such patients?

Information on the criteria that must be met to be eligible for an exemption, along with how to obtain a Performance Year 2025 Exemption Application, can be found here: https://qpp.cms.gov/mips/exception-applications. Please be aware that you'll need a Health Care Quality Information System (HCQIS) Access Roles and Profile (HARP) account to submit a MIPS exception application, and the MIPS exception application deadline for performance year 2025 is 8 p.m. ET on December 31, 2025. The website also provides instructions on how to obtain a HARP account.
