      Measure specification states "Please note that present on admission codes may be those entered by coding staff, extracted from billing/claims data" for both numerator and risk adjustment. 

      Can you please confirm the data source for reporting is based solely on the submitted QRDA I file or are present on admission codes also pulled from the billing/claims data?
