Thank you for your inquiry regarding CMS153v13 (2025 Performance Period). This measure requires a chlamydia test, represented using the QDM datatype "Laboratory Test, Performed" using any codes from the "Chlamydia Screening" (2.16.840.1.113883.3.464.1003.110.12.1052) value set, documented in a structured field in the EHR, has a non-null result, and approved for entry by a physician for the management of care. A non-null result can be interpreted as a result that is present in the electronic record in which any entry is acceptable; no specific value or code is required. We are not able to provide guidance on how these events should be documented in the EHR. We recommend you consult with your EHR vendor and clinical partners.

If you have questions about understanding measure requirements, you may refer to the "Guide for Reading eCQMs" for additional guidance. If you have questions regarding implementing the measure, you may refer to the "Implementation Checklist for eCQM Annual Update" for additional guidance. These resources can be found in the eCQI Resource Center: https://ecqi.healthit.gov/ep-ec?qt-tabs_ep=ecqm-resources&global_measure_group=eCQMs

    • Failure to capture tests being performed external to our EMR/institution

We have labs sent off to LabCorp or other external labs, thus the results are faxed in and therefore not mapped through EPIC (our EMR) as a quantifiable result. Is the only way to appropriately capture this for the provider to manually document the value in their note somewhere and to then map that? or can simply the satisfied order (without EPIC being able to identify the value) work for this measure? We are trying to find a way to adequately capture the work our providers are appropriately doing, but also while trying to honor their patient's insurance company as well as transportation needs by sending to external labs.
