Thank you for your inquiry regarding CMS819v4, Hospital Harm - Opioid-Related Adverse Events. This measure assesses the number of inpatient hospitalizations for patients age 18 and older who have been administered an opioid medication outside of the operating room and are subsequently administered a non-enteral opioid antagonist outside of the operating room within 12 hours. Patient Controlled Administration (PCA) is considered by the measure. In the scenario you mention, the Narcan was administered within 12 hours of the Morphine, therefore the patient would be included in the numerator.

This is the question.  Would a patient receiving Morphine PCA with an order of: Morphine 60mg in 0.9% sodium chloride 60 ml (1mg/ml) with order reading MS 1 mg lockout Q 10 mins, basal rate 1mg/hr with total dose of 7mg/hr . PCA administration began 6/2/2025 @ 1641 was stopped 6/3/2025 @ 0030 due to over sedation. Patient continued to deteriorate, then patient given Narcan 0.4mg 6/3/2025 @ 0636. (PCA stopped < 12 hours at narcan administration time.) Would the PCA administration of morphine be considered in this situation or is the measure looking at opioids given at a discrete time vs PCA administration?
