      Thank you for inquiring about version 13 of the Documentation of Current Medications in the Medical Record eCQM. For version 13 of the measure, age is included, and patients must have been 18 or older at the start of the measurement period. Although the narrative language for the initial population states "All visits occurring during the 12-month measurement period for patients aged 18 years and older,” the logic notes "where AgeInYearsAt(date from start of "Measurement Period")>= 18.” Therefore, if we are discussing calendar year 2024, patients must have turned 18 by December 31, 2023.

      Starting with version 14 of the measures, the age restriction has been removed, and the initial population is now based solely on the presence of a qualifying encounter where current medications are expected to be documented.

      For CMS68 Documentation of Current Medication

      The measure speciation reads that patients must be 18 years and older to be included in this measure.

      My questions are:

      1.)Does the patient have to be 18 years by the start of the measurement year to be included? 

      OR

      2.) Can the patient turn 18 years in the middle of the measurement year and be included?

       

      NCQA PCMH and our vendor are giving us two different answers.
