If patients have not completed the preop survey by the time they arrive for their procedure, we are implementing completion by the Preop prep RN on the day of surgery.  Does the time of this documentation in the EHR need to show as being recorded before the actual start time for the procedure?   For example, patient in preop and survey recorded by RN @ 0831; patient into the OR @ 0847.  

Our concern is the nurse may verbally complete the survey with the patient prior to surgery but may not document the survey question responses electronically until after the pt leaves the room for the procedure and the nurse has time to complete the record.  Ideally, this will not happen, but realistically workflow wise, it could.  

I do not see time the survey is recorded listed in the Data Elements but want to be sure as long as the preop survey is recorded on Day 0 of the procedure, we are still in compliance with the measure requirement.
