Note that the measure does not prescribe specific workflows for measure reporting; therefore, you will need to review the rules configured in your measure reporting system to understand how open versus closed encounters are reflected in your providers’ performance scores. The measure specification specifically defines the qualifying encounter relevant period—this includes the clinically relevant start (admission) and end (discharge) dates and times for the patient encounter (whether a record is open or closed is not considered). Other timing elements include a screening that occurs during the qualifying encounter or within the 14 calendar days prior to that encounter, and AND if positive, a follow-up plan must be documented on the date of or up to two calendar days after the encounter. According to the measure’s Guidance section, “An example to illustrate the follow-up plan documentation timing: if the encounter is on a Monday from 3-4 pm (day 0) and the patient screens positive, the clinician has through anytime on Wednesday (day 2) to complete follow-up plan documentation.”

      Hello, I have a question about how encounter closure affects the Depression Screening & Follow-Up Plan measure. Specifically, does the encounter in which the screening was completed need to be closed to satisfy the measure? If so, is there a specific timeline for its closure?
