This measure requires documentation of an HbA1c or GMI test using QDM datatype "Laboratory Test, Performed", identified by the "HbA1c Laboratory Test" (2.16.840.1.113883.3.464.1003.198.12.1013) value set or the "Glucose Management Indicator" LOINC code (97506-0), along with the test result and date. A patient reported test can be counted towards HbA1c or GMI testing provided that the results are sent to a laboratory or provider's office for analysis.     • Clarification on A1C documentation

      • I have a practice that enters and tracks this for their patients due eye complications, but does not order the labs. They usually retrieve the data from the PCP reports, from the patient and/or patient devices. Is this acceptable for this measure?
