Need clarification for building and tracking with new EMR system.
It is clear to me that if NO VTE prophylaxis is established, we need to document why there is a contraindication to BOTH mechanical AND pharmacological.
It is also clear to me that ambulation alone is not a reason not to administer.
What is not clear to me is that if we document and administer pharmacological prophylaxis, do we need to also document and administer mechanical prophylaxis in high-risk patients? Is one method sufficient to be included in the numerator for this measure?
If we are ordering one method, does there need to be documentation as to why we are not using both?
For example: Patient is getting a pharmacological prophylaxis but there is no mechanical prophylaxis ordered or documented reason why there is no mechanical prophylaxis ordered. Does this count toward numerator and not considered a fall out?
If this is considered a fall out, what would need to be documented to suffice?
