      Thank you for your inquiry regarding CMS122v13 (2025 Performance Period). This measure's denominator looks for those patients who have a qualifying encounter and an active diabetes diagnosis using a code from the "Diabetes" value set (2.16.840.1.113883.3.464.1003.103.12.1001) during the measurement period. If both the onset and abatement dates are missing, the patient will not be eligible for the measure since there is no documented evidence of a diagnosis of diabetes. We are not able to provide guidance on how these events should be documented in the EHR. We recommend you consult with your EHR vendor and clinical partners.

      I have a question regarding condition onset and stop dates.

      For onset dates that are unknown, do we use the date that the patient was first seen for the condition/symptom?

      Regarding conclusion dates such as for a fall, since this is considered an event and not a condition, do we conclude the fall the same day and if the fall led to other symptoms such as pain or fracture these would have their own start and stop dates, correct?
