This measure requires documentation of an HbA1c or GMI test, identified by the "HbA1c Laboratory Test" (2.16.840.1.113883.3.464.1003.198.12.1013) value set or the "Glucose Management Indicator" LOINC code (97506-0), along with the test result and date. A patient reported test can be counted towards HbA1c or GMI testing provided that the results are sent to a laboratory or provider's office for analysis.       This is regarding the 2025 MIPS performance period.
       
      While the specification sheet doesn't explicitly state not to use patient-reported HbA1c or GMI levels (like it does in the CQM specs), it does specify in the definition for glycemic status assessment that the laboratory test was performed which we would need to be able to verify.
