      The QDM does not prescribe the source of diagnosis data in the EHR. Dementia diagnoses may be found in a patient’s problem list, encounter diagnosis list, claims data, or other sources within the EHR. We recommend working with your EHR vendor to map dementia diagnoses documented in the EHR to the QDM Diagnosis datatype.

      The exclusion definition above shows that the dementia diagnosis prevalencePeriod (or time from diagnosis onset to abatement) “overlaps before” the ED encounter relevantPeriod (or the time the encounter began to the time the encounter ended). Meaning, the patient’s dementia diagnosis needs to start before the ED encounter begins for the encounter to be excluded from the measure denominator. Since the time stamp in the inquiry records the dementia diagnosis at 10:42, which is after the start of the ED encounter relevantPeriod (10:26), this particular case does not meet the denominator exclusion.

      Will diagnosis of advance dementia acceptable when it is listed   as one problem  during ED encounter documented in ED provider notes time stamped platform/document. it also interfaced or populate to inpatient encounter problem list. My question will the ED provider document "time stamped acceptable to exclude. Example Pt in ED @ 2/10 @ 10:26, ED provider notes Time stamped @10:42 with Dementia listed on problem list.  Inpatient encounter start at 11:49 (pt out of ED). Can we pull from ED provider notes with time stamped document to map problem/diagnosis for exclusion.  As per specs state at some point during encounter with the following conditions- dementia Thanks
