Thank you for your inquiry regarding CMS832v2, Hospital Harm - Acute Kidney Injury. You are referencing logic for one of the Denominator Exclusion criteria: Inpatient hospitalizations for patients with an increase in serum creatinine value of at least 0.3 mg/dL between the index serum creatinine and a subsequent serum creatinine taken within 48 hours of the encounter start. The logic definition for this exclusion, "Encounter with 0.3 mg dL or More Increase in Creatinine within First 48 Hours", includes an if-then statement where if "Increase of 0.3 or More Using Lowest Creatinine within 24 Hours" does not exist, the logic looks for "Increase of 0.3 or More Using First Creatinine within First 48 Hours". The if-then statement allows for a baseline creatinine result within the first 48 hours of a hospitalization, the value for LowestSerumCreatinine in the first 24 hours or EarliestSerumCreatinine within the first 48 hours.

    • When the lowest creatinine value for a hospitalization exists after the first 24 hours, the Denominator Exclusion statement "Increase of 0.3 or More Using Lowest Creatinine within 24 Hours" can never be met.

"Increase of 0.3 or More Using Lowest Creatinine within 24 Hours" states that the Index Creatinine Lab must be resulted within the first 24 hours of hospitalization. It also states that there must be equivalency between "LowestSerumCreatinine" and the Index Creatinine value.

Because LowestSerumCreatinine is compiled from the entire hospitalization, this Denominator Exclusion statement will never be met with the lowest Creatinine result for the hospitalization is after the first 24 hours.
