Thank you for your question regarding CMS871v4, Hospital Harm - Severe Hyperglycemia. For this measure, hospital days are 24-hour periods that start at the time of admission to the hospital, which includes time in emergency department (ED) and/or observation status when the transition between discharge from these encounters and admission to the inpatient encounter is one hour or less. Using your example, the ED start date and time (15:51 on May 11) would be considered as the start of the inpatient hospitalization period since it was less than one hour between the discharge from the patient’s ED encounter and the start of the patient’s inpatient admission status. Please see the example table for an illustration of how the measure would determine eligible hospital days based on a hospitalization period start time of 15:51 on May 11.

      Dear JIRA,

I have a question on the timing of Days in period function.  Does the 24 hour timing start with arrival time or admission to inpatient time.

The specifications include the ED and observation class if there is less than on hour between them and the inpatient class.

We need to understand if the timing of the 24 hour “days” starts with arrival or admission time.

      For example:

Patient arrives in the ED on 5/11 at 1551.

Pt is admitted to inpatient status on 5/12 at 1837.

(Less than 1 hour between ED and inpatient)

Which time is used to calculate the days?  5/11/25 1551 or 5/12/25 1837?

      Thanks,

      Kim
