We have updated the narrative for the measure’s denominator exclusion and numerator exclusion criteria for improved clarity in the latest version of the measure, CMS871v5, Hospital Harm - Severe Hyperglycemia (https://ecqi.healthit.gov/ecqm/hosp-inpt/2026/cms0871v5 ), for use in the 2026 reporting period.

      I have a question about the human readable section.  Under the Denominator exclusions it reads “patients who have a discharge Diagnosis to Home or to a health care facility for hospice care”.

      I believe this should read “patients who have a discharge Disposition to Home with hospice or to a health care facility for hospice care”.

      CMS Discharge codes:

      Discharge to Home is 01

      Discharge to home with hospice is 50

      Discharge to Hospice medical facility is 51
