      Thank you for your question regarding CMS871v4, Hospital Harm - Severe Hyperglycemia. This measure’s initial population includes inpatient hospitalizations for patients age 18 and older that end during the measurement period where there is a presence of at least one glucose value >=200 mg/dL at any time during the encounter. Assuming that the patient described in your question is 18 or older and their hospitalization ends during the measurement period, the patient’s inpatient hospitalization would meet the measure’s initial population and denominator criteria, as the inpatient hospitalization has the presence of a glucose value >=200 mg/dL during the encounter (on day 25 of the hospitalization).

      Unlike the measure’s numerator criteria and two measure observations, the measure’s initial population criteria is not restricted to the first 10 days of a hospitalization. Please see the part of the associated initial population logic, below, showing that this part of the initial population criteria looks for a qualifying hospitalization with the presence of a glucose value >= 200 mg/dL where the glucose lab test was performed anytime during the hospitalization period (or encounter).

      I have a question about the human readable AND the Written Logic.

      For the initial population, it says: Administration of at least one dose of insulin or any hypoglycemic medication that starts during the encounter; or

      • Presence of at least one glucose value >=200 mg/dL at any time during the encounter.

       

      If a patient is in the hospital for 30 days and does not have a diagnosis of diabetes but ONLY has a blood glucose of > 300 or > or equal to 200 on day 25 – Is this patient in the Initial population / denominator?

      (Or are these values restricted to the first 10 days?)
